Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. The covered entity described in this notice is Denver TMS Therapy, a trade name of TMS Therapy Centers of Colorado.

Treatment. We use your health information to provide, coordinate, and manage your care, including with other clinicians involved in your treatment.

Payment. We use and disclose health information to verify benefits, obtain prior authorization, and bill and collect payment for services.

Health care operations. We use health information for quality review, training, care coordination, and administrative functions.

As required or permitted by law. We may disclose health information when required by law, for public health activities, health oversight, judicial proceedings, law enforcement purposes, to avert a serious threat to health or safety, and for workers' compensation, as permitted by the HIPAA Privacy Rule and Colorado law.

With your authorization. Other uses and disclosures, including most uses of psychotherapy notes, marketing, and any sale of health information, require your written authorization, which you may revoke in writing at any time.

You have the right to inspect and obtain a copy of your records; to request an amendment; to receive an accounting of certain disclosures; to request restrictions on uses and disclosures; to request confidential communications by alternative means or at an alternative location; to request a paper copy of this notice; and to be notified of a breach of unsecured protected health information.

We are required by law to maintain the privacy of protected health information, to provide this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We may change this notice, and changes will apply to information we already hold; the current notice will be posted on this page.

You may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
